This document describes 1-800 Notify’s policy regarding information received about you during your phone call to the IVR Autopay by Phone service. We believe you should know what data we collect from you and how we use it. We believe you should be able to make decisions about the information that you share with us.

1. Information You Share With Us
Depending on the way your organization has designed the IVR Autopay Service, we may ask for your account number (or a portion of it), to identify you. Then, in order to authenticate or verify your identity, we may ask for one of the following pieces of personal information: date of birth, street number or PO Box of your billing address, ZIP Code of your billing address, phone number tied to your account, or last 4 of the social security of the account holder.
Additionally, when you choose to make a payment on our Service, you may enter your Credit Card, Debit Card, or Bank Account information to complete the payment. We will only share this information with the payment processor that is selected and approved by your healthcare organization.
2. Additional Information We Receive About You
Since you are using a telephone to call into the IVR Autopay Service, we may be able to see the phone number of the telephone you are using to call into the Service. If you have blocked your Caller ID display phone number, we will not have that information.
3. Information We Share and Disclose
We share the information you provide with your healthcare organization that has contracted with 1-800 Notify to provide the IVR Autopay by Phone services. Additionally, if you choose to make a payment on the Service using a Credit Card, Debit Card or Bank Account, we will share that information with our payment processor, who is selected and approved by your healthcare organization. We do not store your payment information anywhere on our servers. We will not disclose or supply this information to any other party.
4. Managing Your Personal Information With Us
If you find that your personal information used to identify and authenticate yourself, or your account balance or related financial information presented on the IVR Service is not accurate, you must contact your healthcare provider directly to correct this information.
Deletion of Log Data - We keep call log data up to a maximum of 18 months, and may, upon request of our clients (your healthcare organization) reduce the storage duration of logs for your particular IVR Autopay by Phone service.
5. Changes to This Privacy Policy
We may revise this privacy policy from time to time. Our most current version of this policy will always be at https://1800notify.zendesk.com/hc/en-us/articles/360033223771. By using our service, you agree to be bound by this Privacy Policy.
