<appt_day> = day of week of appt

<appt_date_mmmmdd> = October 25 (month spelled out fully)

<appt_date_mmmdd> = Oct. 25 (month in 3 digit abbrev with period and space)

<appt_date_mmdd> = 9/25 (no leading zero)

<appt_time_h_min_AM> = 9:25 AM (no leading zero, uppercase AM/PM)

<provider> = provider name

<name> = patient name

TIP: If make text too long (over 160 characters), then the phone # at end could be cut into two messages and no longer “clickable” to be easy to call back... so it’s a good idea to keep your text message short!
